It started out as a 100 hour practicum in a graduate cultural diversity class at DePaul, for which he had to select an unfamiliar group of people who were medically underserved. The author is now servicing this shelter full time. He presently runs the shelter's health care case management program. He has finished his graduate studies as an adult nurse practitioner and hopes to obtain a collaborative agreement with a physician to provide a wider range of primary services for the men at the shelter. He is also working with the shelter staff on a proposal for a homeless respite program, which would include having a 24-hour place (possibly an apartment) for the homeless who are in between hospital discharge and the shelter and need a place to recover.

A Paradigm Shift in Housing and Homeless Services: Applying the Population and High-Risk Framework to Preventing Homelessness - Free Access

FREE Access to Full Text: This literature review examines the new field of homeless prevention for practices that show promise for preventing homelessness among at-risk individuals. In 1994 the Federal Interagency Council on Homelessness identified inadequate discharge planning as a significant factor contributing to homelessness among persons with mental illnesses and/or substance use disorders. This study is related to a strategy in the plan that recommends identifying and promoting the use of effective, evidence-based homelessness prevention interventions. The resulting statewide prevention plan identifies existing institutional barriers that can contribute to or exacerbate homelessness and makes recommendations on specific problems or barriers identified within existing programs. These recommendations are rooted in the vision of the council to develop statewide systems and policies that forge partnerships among state agencies that allow communities to achieve local solutions to homelessness.
